Proceed to Post Office with the following and request "Hand Carried"

• 2 Recent passport-size photographs
• 1 Expired passport
• 1 Proof of identification (drivers license and copy)
• 1 Proof of travel within 14 days (itinerary)
• 1 Completed DS 11 application form (obtained from the Post Office)
• 1 Completed Travco Authorization form

Passport Renewal for Minor

Proceed to Post Office with the following and request "Hand Carried"

• 2 Recent passport-size photographs
• 1 Proof of citizenship original birth certificate
• 1 Expired passport
• 1 Proof of travel within 14 days (itinerary)

Both parents must be present with ID and the minor present (if one parent is absent they must provide a copy of their identification along with a notarized statement of consent)

• 1 Completed DS 11 application form
• 1 Completed Travco Authorization form

Replacement of a Lost or Stolen Passport

Proceed to Post Office with the following and request "Hand Carried"

• 2 Recent passport-size photographs
• 1 Proof of citizenship (Expired passport, or original birth certificate, or original naturalization certificate)
• 1 Proof of identification (drivers license and copy)
• 1 Proof of travel within 14 days (itinerary)
• 1 Completed DS 64 application form
• 1 Completed DS 11 application form
• 1 Completed Travco Authorization form

First Time Passport Issuance (Adult):

Proceed to Post Office with the following and request "Hand Carried"

• 2 Recent passport-size photographs
• 1 Proof of citizenship (Expired passport, or original birth certificate, or original naturalization certificate)
• 1 Proof of identification (drivers license and copy)
• 1 Proof of travel within 14 days (itinerary)

All consulate fees are Additional. We also supply necessary forms plus list of requirements, estimated total cost and number of working days needed for processing. For prompt and accurate service, state purpose of trip, number of persons and passports, and nationality in appropriate circumstances.

A note of explanation together with a copy of the itinerary and name to contact for whatever reasons may be necessary. These should accompany all passports for processing of visas.
